Review Recency Matters More Than Review Count

If your competitor got 20 reviews in the last 3 months and you got 2, Google sees them as more actively trusted right now — regardless of your total count. Review velocity (how fast you earn new reviews) is a direct ranking signal. Stale reviews, no matter how many you have, lose to a competitor with consistent recent ones.

Their Google Business Profile Is More Complete

Google rewards completeness. If your competitor has filled in every service, added 50+ photos, answered every Q&A question, and posts regular updates — and your profile has the basics only — theirs is considered more authoritative. Google uses completeness as a proxy for how active and trustworthy a business is.

Their Website Backs Up Their Ranking

Your Google Business Profile doesn’t rank in isolation. Google looks at the website linked to your listing as additional confirmation of your relevance and authority. If their website loads faster, has better local content, or has more external sites linking to it — that pushes their map listing up even if your reviews are higher.

They’re in a Better Location (Relative to the Searcher)

Google Maps results factor in proximity. If their clinic is physically closer to where most searches in your area originate — the town center, the main hospital, or a dense residential area — they get a geographic advantage. But strong optimization can overcome proximity in most competitive markets.
